Acknowledgment of Risks:
I, the undersigned, as the parent/legal guardian of the minor participants[listed at the bottom of this form], understand that participation in pottery activities and classes at the Studio involves inherent risks, including but not limited to the use of tools, equipment, and materials that may result in personal injury or property damage. These risks may include, but are not limited to, cuts, burns, muscle strains, slips, and falls.
Waiver of Liability: I hereby voluntarily waive, release, and discharge Mud And Easel[c\o Bahlko Inc], its owners, instructors, employees, volunteers, and representatives from any and all claims, liabilities, damages, or expenses that may arise as a result of the minor's participation in any activity at the Studio. I agree not to hold Mud And Easel[c\o Bahlko Inc] liable for any personal injury or property damage that the minor may incur.
Assumption of Responsibility: I accept full responsibility for any injury or damage that may occur as a result of the minor's participation. I acknowledge that the minor will follow all instructions provided by Studio staff and adhere to all safety protocols outlined by Mud And Easel[c\o Bahlko Inc].
Indemnification: I agree to indemnify and hold harmless Mud And Easel[c\o Bahlko Inc], its owners, instructors, employees, volunteers, and representatives from any and all claims brought against them as a result of the minor's actions, omissions, or negligence during participation.
Adult Guardian Presence and Supervision: I understand that my presence or the presence of an appointed adult guardian is required during the class. I acknowledge that the adult guardian will ensure that the minor follows all rules, guidelines, and instructions provided by the Studio staff.
Medical Treatment: I authorize the Studio to obtain medical treatment for the minors in case of an emergency and I agree to be responsible for any costs incurred for such treatment.
Photography/Videography Consent: I consent to the use of any photos or videos taken during the minor's participation for promotional purposes.
Acknowledgment of Understanding: I have read this waiver, fully understand its terms, and sign it voluntarily on behalf of the minors. I understand that by signing this document, I am waiving certain legal rights for the minor, including the right to sue the Studio for any injury or damages.
